Welkom bij TerraPathos!

In deze algemene voorwaarden wordt beschreven welke overeenkomsten gelden in de praktijk en op deze website.

In de praktijk

Definities

Hieronder wordt met cliënt zowel de persoon bedoeld die de overeenkomst aanvraagt voor zichzelf of een ander, alsook de personen die samen gebruik maken van de ondersteuning bij TerraPathos. Dit kunnen ouders en kinderen zijn, partners die samen een sessie volgen, werkgevers en werknemers, ...

Met counselor wordt hieronder Tom Pattyn bedoeld, los van de ondersteuningsvorm die aangeboden wordt.

De overeenkomst is wat er tussen cliënt en counselor overeen wordt gekomen ivm het doel van de samenwerking, de kostprijs, de planning, etc...

Onder diensten wordt alles verstaan dat de counselor uitvoert ten dienste van de overeenkomst. (counseling, gesprekken, oefeningen, administratie, ...)

De overeenkomst start vanop de dag dat de eerste afspraak wordt vastgelegd.

Algemene afspraken

De Algemene Voorwaarden worden bij de start van de Overeenkomst vermeld aan de Client in het intakeformulier en staan vermeld op de website www.terrapathos.be
Door de overeenkomst aan te gaan, aanvaardt de cliënt deze Algemene Voorwaarden.

Diensten

De counselor biedt professionele zorg aan in de vorm van individuele counseling of coaching, gericht op het bevorderen van het welzijn van de cliënt en zijn systeem.

De diensten wordt afgestemd op de specifieke wensen en behoeften van de cliënt, in overleg met de counselor.

De counselor kan niet garanderen dat de behandeling altijd leidt tot het gewenste resultaat.

Duur en frequentie van de diensten

Een standaard sessie duurt 60 minuten, tenzij anders overeengekomen.

Het aantal sessies en de frequentie worden in overleg met de cliënt bepaald, afhankelijk van de behoefte van de cliënt.

Tarieven en betalingen

De tarieven voor een sessie zijn gebaseerd op de duurtijd :

• 35 euro voor 30 minuten

• 65 euro voor 60 minuten

• 95 euro voor 90 minuten

Betalingen gebeuren cash of via bankapp. Per overschrijving betalen kan enkel bij langdurige trajecten, mits goedkeuring van de counselor.

(Gedeeltelijke) Terugbetalingen via een ziekenfonds zijn afhankelijk van de voorwaarden van dat ziekenfonds en worden door de cliënt zelf geregeld. De counselor vult op aanvraag de formulieren in van het ziekenfonds.

Indien de cliënt recht heeft op vergoeding van de zorgverzekeraar, is het de verantwoordelijkheid van de cliënt om contact op te nemen met de zorgverzekeraar en te zorgen voor de juiste aangifte. Een weigering van uitbetaling van de zorgverzekeraar kan in geen geval leiden tot een terugbetaling van de betaalde sessies.

Voor sessies op verplaatsing wordt geen extra kost aangerekend, indien de verplaatsing minder dan 10km bedraagt (vanuit Gestelstraat 52, 2250 Olen).
Indien de verplaatsing meer dan 10km bedraagt, kan overeengekomen worden de verplaatsing te vergoeden naar gebruikelijk tarief per half of volledig uur die de verplaatsing in beslag neemt.

Annulaties en niet opdagen

Een afspraak annuleren dient minimaal 24u op voorhand te gebeuren. Indien een afspraak niet tijdig geannuleerd wordt, is de counselor gerechtigd deze volledig of gedeeltelijk aan te rekenen.
Bij ziekte van de cliënt kan een afspraak ook geldig geannuleerd worden door het doormailen van een doktersattest.

Gebruik van de website

Door het gebruik van deze website worden geen persoonsgegevens verzameld of opgeslagen via cookies of andere technieken. De website verwerkt geen persoonlijke informatie van bezoekers.

Overige bepalingen

Wanneer een bepaling uit deze voorwaarden geheel of gedeeltelijk in strijd zou zijn met een wettelijke bepaling, en daardoor nietig zou zijn, dan blijven deze voorwaarden voor het overige onverminderd van kracht. Indien nodig zullen partijen gezamenlijk een nieuwe bepaling overeenkomen die in lijn ligt met het doel van de nietige bepaling.

De counselor is bevoegd om deze Algemene Voorwaarden eenzijdig te wijzigen. De laatste versie van deze algemene voorwaarden zijn steeds van toepassing op nieuwe Overeenkomsten. De recentste Algemene Voorwaarden zijn altijd terug te vinden op de website.

De counselor is niet gebonden aan kennelijke materiële vergissingen, typfouten, rekenfouten of verschrijvingen zoals bv een onrealistisch hoge korting of ongebruikelijk lage vraagprijs.

2. Er worden regelmatig evaluatiemomenten (tijdens therapiesessies) ingelast waarbij het verloop van de therapie expliciet wordt besproken en zo nodig bijgestuurd. Je kan op elk moment de therapie beëindigen. Dit gebeurt in onderling overleg. Ter afronding worden doorgaans één of meerdere sessies voorzien om de samenwerking te evalueren en gevoelens in verband met afscheid een plaats te geven.

3. Wanneer de Cliënt wenst gebruik te maken van het herroepingsrecht is de Cliënt verplicht om dit minimaal 24u en liefst 48u voor de ingeplande sessie kenbaar te maken aan Evi Debruyne en dit telefonisch via 0477 87 53 11.

4. In geval van laattijdige annulering door de Cliënt, is Evi Debruyne gerecht om 50% van de waarde van de Diensten aan te rekenen. Bij Afwezigheid zonder verwittigen wordt de volledige waarde van de dienst aangerekend.

5. Indien Evi Debruyne door overmacht de Diensten niet kan uitvoeren, zal Evi Debruyne zo snel mogelijk daarover contact opnemen met de Cliënt en wordt de Dienst in overleg met Cliënt verplaatst naar een later tijdstip.

6. Evi Debruyne is op geen enkele wijze gehouden aan enige vorm van schadevergoeding voor het annuleren van een activiteit/sessie.
